Chennai, June 26, 2026: In a remarkable medical case that highlights how stroke can sometimes present without the warning signs, doctors at Iswarya Hospital, OMR, Chennai successfully diagnosed and treated a 28-year-old man who suffered an acute ischemic stroke despite showing no obvious neurological symptoms. The patient, a young smoker who had recently travelled from Singapore, experienced what he described as a “strange sensation” affecting the left side of his body shortly after returning from Singapore. Unlike the common symptoms associated with stroke, he had no weakness, facial drooping, slurred speech, difficulty walking, or impaired limb movement.

Initially, the patient consulted a physician in Chennai, where a routine examination revealed no obvious abnormalities, and he was reassured. However, when the unusual sensation persisted, he sought further evaluation at Iswarya Hospital, OMR, Chennai in April 2026, where he was examined by Dr. Deepa Avadhani, Senior Consultant Neurologist. Despite the patient’s completely normal neurological examination, Dr. Deepa made the critical decision to investigate further.

An urgent MRI brain scan revealed an acute right thalamic infarct — a type of ischemic stroke affecting the brain’s sensory processing center. The patient was immediately started on antiplatelet therapy, statins, and neuroprotective medications. Over the course of a week, his symptoms gradually improved, and he was discharged in stable condition with complete neurological recovery.

Speaking about the case, Dr. Deepa Avadhani, Senior Consultant Neurologist at Iswarya Hospital, said: “Stroke is often associated with dramatic symptoms such as paralysis, slurred speech, or collapse. However, this case reminds us that stroke can sometimes present very subtly. In this patient, the stroke affected the thalamus — an area responsible for sensory perception rather than movement or speech. That is why the only symptom was an unusual, altered sensation, which could easily have been overlooked.”

“The thalamus acts as the brain’s sensory relay center, processing touch, temperature, and body awareness. When this region is affected by stroke, patients may experience abnormal sensations without weakness or visible disability, making diagnosis particularly challenging. “added Dr Deepa

“This case serves as an important reminder that stroke does not always announce itself dramatically. Sometimes, it begins with subtle symptoms that feel unusual but are easy to dismiss. Early recognition and immediate medical attention can make a life-changing difference, especially in younger patients who may not perceive themselves to be at risk.” Said Dr. S Rajasundaram, CEO, Iswarya Hospital

The successful recovery of this young patient was made possible through clinical vigilance, timely diagnosis, and immediate medical intervention, reinforcing the importance of paying attention to unusual neurological symptoms, regardless of age.
