Is Your Health at Risk? The Secret Link Between a Common Parasite and Cervical Cancer

In the most remote corners of the world, health crises often sneak by unnoticed, lingering in the shadows of everyday life. One such silent threat, a common parasitic infection, Schistosoma haematobium, is not only known for causing urogenital schistosomiasis but has now been linked to a significant rise in cervical cancer risks. As shocking as it sounds, this parasite, often found in women living in areas with poor sanitation and limited access to clean water, has shown troubling genetic effects that could change how we think about women’s health. This new research is opening our eyes to a whole new set of dangers and calls for urgent action.

Schistosoma haematobium, while often recognized for its role in causing bladder cancer, has largely flown under the radar when it comes to cervical cancer. But recent research has brought new attention to this underexplored connection. Presented at the ESCMID Global 2025 conference, this study sheds light on how the infection could alter the cervix at the genetic level, possibly increasing the risk of cervical cancer.

Cervical cancer has long been associated with human papillomavirus (HPV), but what if there are other factors at play? In a study conducted on women in Tanzania, researchers focused on 39 participants, 20 infected with S. haematobium and 19 uninfected controls. They found disturbing genetic changes in the cervical tissue of women who had the parasite, which could heighten their susceptibility to cancer. These changes were particularly evident after treatment with praziquantel, the standard antiparasitic medication.

What makes this discovery so alarming is the scale and nature of the genetic changes triggered by S. haematobium. Through RNA sequencing, the research team found nine genes that showed significant differences between infected and uninfected women. Even more startling were the 23 genes that had shifted after treatment with praziquantel, and 29 genes that showed further alterations when comparing post-treatment to never-infected individuals.

These genes are deeply involved in processes that govern cell growth and survival, critical factors in cancer development. Key genes such as BLK proto-oncogene, involved in regulating cell growth, and TCL1 AKT coactivator A, known for promoting survival in blood cancers, were found to be disrupted in women treated for schistosomiasis. These genetic changes don’t just indicate an increased risk of cervical cancer they suggest that even the standard treatment for S. haematobium might trigger processes that make women more vulnerable to cancer in the future.

Here’s where the study takes a more concerning turn. Researchers found that the gene changes observed after praziquantel treatment didn’t just stop at cancer markers, they extended to biological pathways involved in inflammation, blood vessel formation (angiogenesis), and tissue degradation. These are the very processes that can weaken the structural integrity of the cervix, making it easier for HPV to infect and persist in cervical cells. In short, the parasite’s effects may extend far beyond the infection itself, leaving women more vulnerable to the cancer-causing mechanisms of HPV.

This revelation raises a pressing question: could treating schistosomiasis inadvertently open the door for another deadly infection, HPV, to do more damage? Dr. Anna Maria Mertelsmann, the lead author of the study, points out that the genetic alterations after treatment may compromise the cervix’s defenses, setting the stage for an increased risk of cervical cancer.

This research has far-reaching implications. Female genital schistosomiasis (FGS), which is often underdiagnosed and overlooked, may be far more dangerous than we thought. Dr. Mertelsmann calls for greater awareness of this condition, particularly in regions where S. haematobium is endemic. Women previously infected with this parasite, even after successful treatment, could face a long-term risk of cervical cancer due to the lasting genetic effects.

For many women in areas affected by S. haematobium, access to early screening is vital. The study emphasizes the need for continuous monitoring, especially post-treatment, to understand the full scope of how this parasite affects women’s health in the long term.

As researchers dig deeper into the interaction between schistosomiasis and cervical cancer, one thing is clear: prevention must be multi-faceted. While treatment for schistosomiasis remains essential, it should be complemented by additional measures to reduce the risk of cervical cancer, such as the widespread adoption of HPV vaccination. Vaccinating at-risk populations can help prevent the most common cause of cervical cancer, thus serving as a crucial preventive measure against the devastating effects of both haematobium and HPV.

The findings of this study are just the beginning. A larger follow-up study tracking 180 women over the course of a year is underway to validate these results. This will allow scientists to gain a clearer picture of how schistosomiasis interacts with HPV over the long term and whether other interventions can help mitigate the genetic changes seen after praziquantel treatment. Researchers are also exploring adjunct therapies, such as immune-modulating or anti-inflammatory treatments, that might help counteract the harmful effects observed post-treatment.

While this study reveals an alarming link between schistosomiasis and cervical cancer risk, it also opens the door for new opportunities in women’s health. It calls for increased awareness of female genital schistosomiasis and highlights the need for targeted treatments and screenings. At the same time, it highlights the importance of prevention through vaccines and the need for further research to fully understand how parasitic infections impact women’s long-term health. This study reminds us that in the fight against cervical cancer, we must look beyond HPV and address all the potential risk factors, especially those hidden in plain sight.
