Swift and Comprehensive Care Saves Young Patient’s Life at Jupiter Hospital

Prompt and aggressive management led to the young patient being discharged with minimal morbidity and with intact central nervous system (CNS) function.

In a remarkable display of medical expertise and teamwork, Jupiter Hospital successfully treated a 2-year-6-month-old boy who was admitted with severe dehydration, hypovolemic shock, and multi-organ dysfunction. The child’s condition had rapidly deteriorated with symptoms including fever, vomiting, loose stools, lethargy, and a significant decrease in oral intake over the past few days.

"Speaking on the critical case, Dr. Shivahar Vijayrao Sonawane, Consultant – Paediatrics & Neonatal Intensivist said, 'Upon arrival at Jupiter Hospital, we acted swiftly to stabilize the young patient. The situation required immediate and intensive interventions to address the severe dehydration and multi-organ dysfunction. Our first priority was fluid resuscitation to counteract the dehydration, followed by administering inotropes like adrenaline and noradrenaline to manage the catecholamine-resistant shock. To maintain blood pressure, we used vasopressin, and Our Vascular Surgeon Dr Shrikant Ghanvat & Anaesthesia Consultant Dr Bhagyashri Arbi, performed nerve blocks (axillary and femoral) to improve circulation to the limbs. Each step was crucial in stabilizing the patient and ensuring their recovery.'

To ensure adequate breathing and protect the airway, intubation was carried out. The patient also received multiple blood product transfusions to address blood loss and support overall recovery. Continuous renal replacement therapy (CRRT) was implemented by Dr Swati Mane, Consultant - Nephrology, Jupiter Hospital, Pune to manage acute kidney injury (AKI), while N-acetyl cysteine drip was administered to treat acute liver injury. Due to the prolonged nil-by-mouth (NBM) status, total parenteral nutrition (TPN) was provided to meet the child’s nutritional needs.

Thanks to the prompt and aggressive Team PICU, Department of paediatrics provided by the multidisciplinary team, the child’s condition improved significantly. He was discharged with minimal morbidity, without any bed sores, and with intact central nervous system (CNS) function. This successful outcome underscores the critical importance of immediate and comprehensive care in managing severe dehydration and shock, as well as the necessity for a coordinated approach to address multi-organ dysfunction in pediatric patients.
