CARE Hospitals, Hitech City Performs Life-Saving High-Risk Heart Bypass Surgery on 68-Year-Old Patient

Hyderabad, 16th December, 2025: CARE Hospitals, Hitech City, has successfully saved the life of Mr. Gokla Nand, a 68-year-old patient from Delhi, through a highly critical and complex off-pump coronary artery bypass grafting (OP-CABG) procedure. The patient was brought to the emergency department in a near-collapse state with severe chest pain, difficulty in breathing, and cough with phlegm. His oxygen levels were critically low, and he was in severe respiratory distress.
 
On arrival, the patient was diagnosed to be in cardiogenic shock, requiring immediate ventilator support and multiple inotropic medications to stabilize his heart and maintain adequate blood pressure. Coronary angiogram was done immediately which revealed severe triple-vessel coronary artery disease, with critical blockages in all major coronary arteries along with severe left ventricular dysfunction. These high-risk findings necessitated urgent, expert surgical intervention to save his life.
 
This case is one of its kind, where the patient underwent a successful off-pump CABG despite being in cardiogenic shock, supported with multiple inotropes and on mechanical ventilatory support.
 
The surgery was performed using a beating-heart technique by Dr. RaviRaju Chigullapally, Senior Consultant – Cardiothoracic & Vascular Surgery, with support from Dr V. Vinoth Kumar, Senior Interventional Cardiologist and a multidisciplinary cardiac and critical care team. Post-surgery, the patient received intensive care with ventilator support, antibiotics, cardiac monitoring, physiotherapy and specialized recovery support. His condition improved rapidly, and he began breathing comfortably, regained strength, and started walking shortly after surgery. In an impressive recovery milestone, he was discharged in just 6 days, fully stable and recovering well.
 
Speaking about the complexity of the case, Dr. Ravi Raju Chigullapally explained that the patient’s condition required advanced surgical decision-making and precision care. “This case was especially challenging due to the patient’s age, severely compromised heart function and respiratory failure at arrival. Performing a high-risk beating-heart bypass surgery in such a critical condition was possible only because of swift coordination, advanced technology and collective clinical expertise,” he said.
 
“From the moment the patient arrived, his condition was extremely critical. He had severe triple-vessel blockages, very low heart function and was already in cardiogenic shock, which made the situation life-threatening. Our priority was to stabilize him quickly, support his heart, and plan the safest and most effective approach for surgery,” said Dr. V. Vinoth Kumar, Senior Interventional Cardiologist, CARE Hospitals, Hitech City.
