Dr. Deepa Avadhani, Senior Consultant Neurologist at Iswarya Hospital has solved an intriguing case that stresses the need for early neurological examination and cautious use of unregulated medications.

The patient was presented at the Iswarya Brain and Spine Centre with severe insomnia -lack of sleep, anxiety and palpitations. On detailed enquiry he mentioned occasional twitching of muscle. Dr. Deepa Avadhani with her able neurology team at the hospital initiated a detailed clinical evaluation given the unusual combination of symptoms.

The patient was advised to undergo elevtromyography, a procedure that displays the characteristics of peripheral nerve hyperexcitability. Nerve hyperexcitability is a state where the nerves transmit excessive signals to the muscles, resulting in persistent twitching. Further investigation revealed the presence of CASPR2 antibodies in the patient’s blood, confirming the diagnosis of Morvan’s Syndrome, a rare autoimmune neurological disorder.

During the investigation, the patient was found to have consumed non-prescribed “native” medicines. Such medicines have the potential to induce severe neurological complications in the patient. Prompt treatment with immunotherapy was given to the patient, and supportive medications. Following treatment, his condition improved significantly, and he was discharged in stable condition.

“Symptoms like persistent muscle twitching and severe insomnia are commonly brushed off as minor complaints, but they could potentially be the first symptoms of serious neurological diseases. This case is a perfect example of how timely intervention and treatment can literally be life-changing, said Dr. Deepa Avadhani, Senior Consultant Neurologist at Iswarya Hospital.

Herbal or traditional medicine is considered harmless; it is possible that some of the ingredients in the medication could be made of heavy metals or immune system-altering agents. Patients should seek medical advice before taking such medication. The case serves as a reminder that not all symptoms are harmless, added Dr Deepa Avadhani.
