Faridabad, May 08th, 2026: In a rare and complex case, doctors at Fortis Escorts Faridabad, successfully treated a 33-year-old woman from Bihar who had been suffering for five years from severe pain and loss of bowel control, resulting in significant emotional trauma and social isolation due to an injury sustained during childbirth 5 years back.
The patient had sustained a tear in the perineum - the area between the vaginal opening and the anus. Such tears usually occur during childbirth, when the baby stretches the vaginal opening and are small and repaired easily, however in this case, the tear extended all the way down to the anus which led the woman to lose control of bowel moments.
A complex surgery was performed with high precision under the care of Dr. Anita Soni and Dr. Isha Wadhawan, which lasted for approximately 1.5 hours. The procedure was successful and the patient was discharged in a stable condition within 2 days.
The patient sustained a severe childbirth-related tear 5 years ago, at the time of her second delivery. This tear was large and extended from the vaginal lining through the underlying muscles to the rectal muscles and anal lining, resulting in a single abnormal passage between the vaginal and anal canals. This was a rare and serious complication that left the patient suffering from fecal incontinence, with complete loss of control over bowel movements and the passage of gas. As a result, maintaining personal hygiene became extremely challenging, significantly impacting her physical health as well as her psychological and social well-being. Although an initial surgical repair was attempted at the time of the 2nd delivery, it was unsuccessful, leading to a progressive deterioration in her condition. Unable to access appropriate treatment in her hometown, she eventually sought care at Fortis Escorts Hospital, Faridabad.
On evaluation, doctors diagnosed a long-standing, complete perineal tear measuring approximately 6 -7 cms, affecting the anal lining and surrounding muscles. After detailed clinical examination and pre-operative investigations, the medical team performed a complex surgical repair.
The procedure, which lasted approximately 1.5 hours, involved meticulous layer-by-layer reconstruction of the anal lining, the sphincter muscles responsible for bowel control, and the supporting tissues between the vagina and anus. The presence of significant scar tissue from the old injury added to the complexity of the surgery. However, the team successfully carried out a smooth and precise repair, essential for ensuring long-term recovery. The patient was discharged in stable condition within two days.
Giving details of the case, Dr. Anita Soni, Additional Director, Obstetrics & Gynaecology, Fortis Escorts Hospital, Faridabad said, “This case highlights the critical importance of timely diagnosis and appropriate management of obstetric injuries. While such severe perineal tears are rare, their long-term impact on a woman’s physical health, emotional well-being, and social confidence can be profound if left unaddressed. Increased awareness, skilled obstetric care during childbirth, and timely intervention are key to preventing and effectively managing such complex conditions.”
Dr. Isha Wadhawan, Consultant, Obstetrics & Gynaecology, Fortis Escorts Hospital Faridabad said, “In long-standing cases like this, surgery becomes more challenging due to scar tissue and distorted anatomy. A meticulous, layer-by-layer reconstruction is essential to restore continence and improve quality of life. With the right surgical approach and post-operative care, patients can achieve excellent outcomes even after years of suffering. She is on regular follow up with the team and the wound has been healing well, she is pain free, with good bowel control and renewed confidence. She is grateful to everyone involved for excellent result and compassionate care".
Dr Abhishek Sharma, Facility Director, Fortis Escorts Hospital, Faridabad said, “Cases like these highlight the importance of timely diagnosis and appropriate intervention following childbirth injuries. No woman should have to live in silence with such complications. With the right clinical care, even long-standing cases can be successfully treated, and our team of doctors has proved it by carrying out the surgery so efficiently.”
Complete perineal tears occur in just 0.05% to 0.2% of childbirth cases.
