As a surgical oncologist, my day often begins in the operating theater performing complex head and neck cancer surgeries. But the hardest part of my job isn’t the surgery, it’s telling a young person that his "simple mouth ulcer" is actually advanced cancer.
In India, oral cancer is a national crisis. We lead the world in cases, with over 130,000 new diagnoses annually. The tragedy is nearly 90% of these are preventable, directly linked to tobacco and areca nut (supari).
The “INDIAN” Risk: More Than Just Smoking
We often see “Smoking kills” warnings on cigarette packs, but India’s danger is often "smokeless" products like Gutkha, Khaini, Mawa, and Zarda. When you tuck tobacco against your cheek, you aren't just getting nicotine, you are absorbing over 70 known carcinogens (cancer inducing agents).
Even "plain" Betel nut is a Group 1 carcinogen. It causes Oral Submucous Fibrosis (OSMF), a condition where the mouth tissues stiffen, eventually leading to "lockjaw" and frequently cancer.
The "Big Three" Warning Signs
The most dangerous myth is that "if it doesn't hurt, it isn't cancer, but the fact is Early-stage oral cancer is almost always painless. Watch these:
1. The 2-Week Rule: Any ulcer that doesn't heal within 14 days.
2. Colour Changes: Persistent white (Leukoplakia) or velvety red patches inside the mouth.
3. Restriction: Difficulty swallowing or a reduced ability to open your mouth fully.
The 2-Minute Self-Exam
• Check the Tongue: Look at the sides and underneath (high-risk areas).
• Feel the Cheeks: Search for lumps or "leathery" textures.
• Trace the Gums: Look for unexplained bleeding or growths.
• Neck: Feel for firm, painless lumps along the jawline.
Prevention is better than cure; Early detection saves life
In India 50% of patients arrive at Stage III. At this stage, treatment is aggressive, involving major surgery with reconstruction and chemo-radiation. Though this is possible, it involves expertise and expenses. However, Stage I cancer has a nearly 90% cure rate. Early detection transforms a life-threatening crisis into a manageable minor surgery.
A Call to Action
We are increasingly seeing patients in their 20s. Tobacco "mouth fresheners" are a trap.
Your mouth is the gateway to your health. Don't let tobacco toxins turn it into a gateway for disease. If you see something, consult an oncologist immediately.
